Fields on the sheet

Record completed vaccinations with their actual dates. Copy the next planned date from your veterinarian’s instructions and keep those instructions with the sheet.

What should a completed row describe?

Identify the animal or the exact group treated and write the date on which the event occurred. Copy product and batch details from the reliable treatment record rather than from memory. Record the veterinarian or responsible person and a document reference so questions can be traced to the original source. If a group is used, preserve the list of included animal identities. A broad description such as all young animals can become ambiguous after animals move between herds or are sold.

How should I enter a future date?

Use the follow-up column only when a veterinarian or the relevant professional instruction supplies a date for that animal or group. Mark it as a planned date, not as a completed vaccination. Do not infer a schedule from another farm’s record or from an empty row in this PDF. Products, species, circumstances and applicable rules can differ. Copy the vaccine, interval, route and dose from the agreed animal health plan, keeping the original instructions with the record.

How do I handle uncertain or corrected records?

If a product, date or batch is unknown, leave the field blank and note what needs checking. Ask the responsible professional for the original record before completing it. Keep a legible correction trail rather than rewriting an uncertain event as if it were verified. The person checking the row should add a dated note when the missing source is found. Store invoices, veterinary documents and any required official records in their proper place; the simple log is an aid to organisation.

How can I use the log with HerdMobile?

Record the completed event against the correct animal in the app and compare its date and notes with this paper. Add a reminder separately if an instructed future action needs one; a saved event and a reminder have different purposes. Keep the completed sheet on the farm and enter each animal event separately in the app when you want a digital copy. Record the completed procedure, any later observations and the instructions given, keeping the animal’s history together. Maintain your own backup of the phone’s records after entering an important batch of events.
